Leave of Absence Form

This form should be completed if you wish to request leave of absence for your child during term time for exceptional circumstances only. Please complete this form and return it to the school office at least two weeks prior to the period for which you are requesting leave of absence.

Permission to Administer Medication

The school is only able to give your child medication if you fill in and return this form. We are unable to administer Ibuprofen unless it is a prescribed medication.
